Ideal for patients starting hormone optimization programs and metabolic health improvements, this 6-month optimization program is designed for safe and effective treatment adjustments. It includes personalized wellness services, such as a tailored treatment plan, hormone optimization, and metabolic health support. You'll also benefit from scheduled telehealth health evaluation visits (at 1, 2, 4, and 6 months), lab monitoring (at 6 weeks, 3 months, and 6 months), ongoing medication adjustments, and direct messaging with your provider through the patient portal. Please note that medications or labs are not included (refer to the details below).
Best for patients with more chronic and complex conditions (such as autoimmune disorders, pre/diabetes, hypertension, and medical weight loss), this hormone optimization program is designed as a 6-month optimization journey. It allows for safe and effective treatment adjustments while offering personalized wellness services. The program includes everything available in the Signature program, plus monthly follow-up telehealth health evaluations, closer monitoring, and advanced treatment planning, as well as added medically supervised weight management if needed. Please note that medications or labs are not included (see below).
After optimization
Stabilization phase (after 6 months) - you are optimized! As part of our hormone optimization programs, ongoing care includes follow-up telehealth health evaluations via video or phone every 3-6 months. We provide long-term hormone optimization prescription management with personalized wellness services that include customized medication and lab tests every 6 months. You can also enjoy direct messaging with your provider through the patient portal.

Our hormone optimization programs focus on long-term hormone management, ensuring that medication and lab tests are customized to your needs, with labs scheduled every 3-6 months. Benefit from medically supervised weight and metabolic care, and stay connected through direct messaging with your provider via the patient portal. Please note that this package does not include blood work or medications (see below).
Blood work is essential for health optimization and is available at discounted medical pricing through Quest Diagnostics, especially for those engaged in hormone optimization programs.

For compounded medications not typically covered by insurance, Sage Health & Wellness offers personalized wellness services at discounted cash-pay pricing through trusted pharmacy partners. Their hormone optimization programs provide treatment with the following estimated costs:
Hormone management therapy: ~$80–120/month
Thyroid optimization: ~$100–125/month
Weight management/insulin resistance: ~$80-250/month.
Autoimmune management (including low dose naltrexone if needed): ~$45/month. Additionally, you can schedule a telehealth health evaluation to discuss your specific needs and medication options.
